What is monobenzone cream?

Monobenzone (also known as monobenzyl ether of hydroquinone, MBEH) is a topical depigmenting agent — the only treatment FDA-approved for final depigmentation in extensive vitiligo affecting more than 50% of the body surface area.

It works by selectively destroying the melanocytes — the pigment-producing cells — in normally pigmented skin, so that skin tone evens out with the surrounding depigmented vitiligo patches. The 20% strength is the standard FDA-approved concentration; 40% is a stronger off-label option used in select cases.

Although the original Benoquin brand was discontinued by Valeant/ICN in the United States around 2008 for commercial reasons (not safety), the active ingredient remains FDA-approved and is manufactured worldwide. India is one of the largest current sources of pharmaceutical-grade monobenzone.

Who is it for?

Patients diagnosed with extensive or refractory vitiligo (typically >50% body surface area) for whom repigmentation therapy is no longer practical, and who have been counselled about permanence and lifelong sun protection.

Who is it not for?

Monobenzone is not a cosmetic skin lightener and must not be used on small vitiligo patches or by anyone seeking general skin whitening. Misuse causes permanent, irreversible loss of pigment in unintended areas.

Is it permanent?

Yes. Depigmentation from monobenzone is, in almost all cases, irreversible. Treated skin will require strict broad-spectrum sun protection for life.

Is it legal to import monobenzone into the United States with a prescription?

The FDA’s personal-importation policy permits a personal supply of up to approximately 90 days of an unapproved medication for a serious condition, with a valid prescription and a written affirmation of personal use. Monobenzone for diagnosed extensive vitiligo qualifies under that framework.

What’s the difference between Benoquin, Albaquin and Uniqueen?

All three are brand names for the same active ingredient — monobenzone — manufactured by different Indian companies. Benoquin was the original US brand (now discontinued); Albaquin is Puneet Laboratories’ brand; Uniqueen is Unique Pharma’s brand, available in a micronized formulation and in both 20% and 40% strengths. Clinically they are equivalent at the same concentration.

What’s the difference between 20% and 40% strength?

20% is the standard, FDA-approved concentration — generally produces full depigmentation in 6–12 months. 40% is a stronger, off-label option that may show visible effects in 4–8 weeks but carries a higher risk of skin irritation. Strength should be selected by your dermatologist based on your case.

How long until I see results?

Most patients on 20% monobenzone applied twice daily begin to see lightening of treated areas after 2–4 months, with most of the change occurring between months 4 and 9. Full depigmentation of treated areas typically takes 9–12 months. Faces and arms generally respond fastest; hands and feet take longer.

What are the side effects?

Common side effects include mild skin irritation, dryness, burning sensation on application and occasional rash. Important risks include “satellite” depigmentation at distant sites and “consort vitiligo” (depigmentation in someone with skin contact with the patient). Long term, treated skin is permanently sensitive to UV and must be protected.
